The itch-scratch cycle of neurodermatitis: In what order does Korean medicine break it?
Neurodermatitis is difficult to cure with advice to simply stop scratching. The first step is to reduce the itching itself, rather than testing your willpower to stop scratching
If you are told to simply endure strong itching as if your hands were tied, unconscious scratching will resume during sleep or while concentrating. First, we check if new redness, heat, oozing, or abrasions have appeared on the thickened skin. When active inflammation is flared up, treatment to reduce inflammation and itching must come first for the prevention of scratching to work realistically.
While continuing the use of prescribed topical agents and wound protection as needed, Korean herbal medicine treatment distinguishes whether the skin is currently hot and moist or dry and cracking. For patterns of blood-heat or damp-heat where the skin is red and hot again, the focus is on reducing heat and inflammation; we do not excessively continue this same approach for skin that has been dry for a long time.
The second step is to break the cycle of nighttime itching, scratching, and awakening
Even if it is bearable during the day, you may scratch the same area when your body warms up before falling asleep or when you sweat during sleep. The wounds from scratching create more itching, and waking up makes the senses more sensitive the next day, continuing the cycle. Even if the appearance of the skin has improved, if you continue to wake up during the night, there is still a need for treatment.
Keep the bedroom cool, moisturize immediately after showering, and protect areas frequently touched by the hands with soft cotton materials. Acupuncture is performed in a way that reduces tension in the neck and shoulders and lowers hypersensitive arousal, rather than strongly stimulating the lesion. Korean herbal medicine treatment also reflects the factors that actually move together among nighttime heat, sweat, dryness, and difficulty falling asleep.
Once itching decreases, the next goal shifts to thickened skin texture and dryness
Even if the frequency of scratching decreases, lichenified (thickened) skin does not thin out immediately. During the period when skin lines are thick and hard and dead skin or cracks remain, we increase the proportion of nourishing the blood and moisturizing the skin to hydrate and recover the skin, rather than simply repeating prescriptions to reduce new inflammation.
Skin texture and color may change more slowly than the itching. Using scrubs or strong exfoliants to quickly remove marks and thickness can trigger inflammation and itching again. First, there should be a change where the skin feels less rough to the touch, cracking decreases, and friction from clothing is more easily tolerated.

People who touch the back of their neck when immersed in work or feeling anxious, or those who repeatedly scratch sweaty ankles and groin, may still feel the urge to scratch in those same situations even if skin inflammation is reduced. Baekrokdam examines whether chest tightness, neck and shoulder stiffness, or difficulty falling asleep occur during tension, and whether sweat, moisture, or digestive discomfort lead to worsening symptoms.
For the tension-type, we focus on soothing the liver, relieving emotional stagnation, and restoring sleep; for the damp-heat type where sweat and friction overlap, we add a focus on lowering moisture and heat. In daily life, we reduce friction from clothing and shoes and avoid leaving the skin damp for long periods. This does not mean avoiding all causes, but rather reducing the recurring conditions that restart the cycle even after treatment.
We view the order of improvement as a flow: less itching, less scratching, and then the skin becoming smoother.
Rather than a single itchiness score, we look at whether the time spent scratching before falling asleep decreases, whether new scratch wounds during sleep are reduced, and whether the time to fall back asleep after waking is shortened. Following this, the thickened plaques soften, cracks and dead skin decrease, and the intervals between comfortable days grow longer.
We do not reduce moisturizing and prescriptions all at once just because the early stages of treatment have improved. We first maintain the improvements in itching and sleep, then shift the focus of the next Korean herbal medicine prescription toward the remaining skin texture, friction, and tension. After analyzing the current skin condition, the treatment plan can proceed in this order.
The itch-scratch cycle of neurodermatitis is not a matter of willpower. The intervals of comfort in daily life increase only when the goals of Korean herbal medicine and acupuncture are shifted sequentially to address active inflammation and itching, nighttime arousal, thickened skin texture, and the conditions that trigger the urge to scratch.
